How to place my child here (Referrals)
Considering Placement at USDB? Here's What You Need to Know:
At the Utah Schools for the Deaf and the Blind (USDB), we're committed to supporting students and families. If you're considering USDB as a placement option for your child, here’s what to expect:
Placement Begins with an IEP or 504 Meeting
All students placed at USDB must enter through an IEP (Individualized Education Program) or 504 Plan meeting. This meeting is scheduled by your local school district, also known as the LEA (Local Education Agency).
The LEA is responsible for:
- Creating and implementing the IEP or 504 Plan
- Conducting evaluations and assessments for initial placement
- Considering USDB as one of the placement options during the meeting
USDB Participation is Required
USDB must be invited to attend any IEP or 504 meeting where our services or placement are being discussed. This includes:
- Transitions from early intervention (IDEA Part C to Part B)
- Students moving from another state
- Students transferring between school districts
- Any meeting about evaluation, eligibility, or potential placement at USDB
Important: USDB services may not be included in any plan unless a USDB representative is present and agrees to the services.
Placement & Transition
If a change of placement is being considered, both the LEA and USDB will:
- Participate in the IEP or 504 meeting
- Agree on the meeting time and location
- Work together to create a smooth transition plan for the student
If USDB is selected as the best placement:
USDB becomes the Designated LEA and takes the lead on future IEP meetings
Your school district will continue to be involved in these meetings
